Associated Eye Care’s Shared Care Agreement
Building Partnerships with Referring Optometrists

Associated Eye Care is committed to providing the best possible care for every patient. Part of that commitment is to ensure that patients return to their primary eye care provider for their post-operative care. Preserving this primary eye care relationship benefits all parties, and patients enjoy a high standard of care with a familiar, conveniently located doctor.

The Shared Care Agreement form sets expectations for the patient that two physicians will share the post-operative care responsibilities and assures referring optometrists that their patients will return. This provides continuity of care for the future as well.

The Shared Care Agreement should be completed & signed by the optometrist and faxed to AEC. A returned signed copy will be sent to the optometrist after the AEC physician has received the document.
